Trying to get "Solemnized" in Nevada

Greetings all,
I am trying to conduct my first wedding in the state of Nevada.  However the state requires the attached "affidavit to solemnize marriages" to be signed and notarized by someone in the church.  I have forwarded my request to the church info center and received no response (probably at a bowling tournament).  I have a hard time believing I would be the first person in Nevada to conduct a Dudeism wedding.  Can anyone help me out?  I have attached the link to the form I need notarized.  I can compensate anyone for their time and effort. 

Page 3 needs to be completed by someone other then me

The State of Nevada is trying to hold me down!  I told my fellow dude I would marry him and his old lady and I am going to try not all that hard but I should be able to out smart a state bureaucracy just let me get a few cocktails in me.
